Job Summary:
The Crisis Care Manager will assist in providing intensive treatment and intervention services through the crisis residential unit (CRU) and emergency service department for adults with mental illness and/or mental retardation for reduction of a crisis in their life that places them at risk of hospitalization.
Supervisory Responsibilities:
- None.
Duties/Responsibilities:
- Assists in assessment/linkage and referral of individuals in crisis to appropriate disposition (CRU, commitment, etc.)
- Assists in providing case management during the individuals stay at the CRU (assessments, referrals, linkages, advocacy, transportation, developing treatment plans, documentation, and monitoring).
- Assists and works with community case managers for residents in the CRU.
- Arranges and provides transportation for other services needed during or on discharge from the CRU.
- Provides crisis intervention services for those in the community.
- Assists and participates in interdisciplinary team.
Required Skills/Abilities:
- Excellent knowledge of case management and crisis intervention services.
Education and Experience:
- Bachelors degree and one year's experience working with the mentally ill, those with intellectual disabilities, and/or substance abuse.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
