Student Chapter Information Form
SHRM student chapters are required to update their leader and advisor information annually. Submission of the Student Chapter Information Form (SCIF) is a requirement to maintain your student chapter's active affiliation with SHRM for the upcoming academic year and eligibility to participate in the Student Chapter Merit Award Program.
Due Date
The SCIF is due June 1 each year for the upcoming academic year.
Instructions
Please complete the SCIF by providing the names of your student chapter leaders and other pertinent information. Please use full names, addresses and formal titles (e.g., John Doe, Ph.D., SHRM-SCP, Professor) and complete the form as completely as possible. Required fields are marked with an asterisk (*).
Begin your form here. A copy of the completed form will be sent to the e-mail address for the Primary Advisor listed.
Please notify SHRM immediately of any changes in your chapter information during the academic year.
