Purpose/Objective

The company reserves the right to prospectively add, change, delete or modify the benefits offered to employees as it deems appropriate. This policy is only a general summary of the health care benefits offered. Employees should consult the applicable plan document for information regarding eligibility, coverage and benefits. It is the plan document that ultimately governs employee entitlement to benefits.

Eligibility

Procedures
